media-video/ffmpeg: update SRC_URI, add missing patches

We now use the new ffmpeg-patches repo for fetching patch files instead of using a custom git repository. This should enhancen transparency regarding what these patches do and makes maintenance a bit easier.

Also, missing patch files are added, which were forgotten to be added in the last media-video/ffmpeg commit.
